Our Personalized Approach To Care

Welcome to Visiting Companions & Caregivers of New Jersey

Every family has a unique situation, so every family receives a personalized care plan.

All our care programs begin with a level of care assessment by a nurse who specializes in chronic illness management and post-hospital, post-rehab care.

A professionally coordinated care program will be designed by the nurse in collaboration with the patient, family, and the patient’s healthcare team.

Our care programs are flexible and can be provided inside a retirement community, at a private residence, alongside Medicare-skilled therapy at home, or can begin after discharge from skilled therapy.
